Before we start - have you read the Patient Information Leaflet (PIL) for this medicine?

The leaflet explains how to use it, who should not use it, the warnings about other medicines, and the side effects. Ask us if you do not have a copy.

Who will be using this medicine (the intended user)?
Are you - the person placing this order - 18 or over?

You must be 18 or over to buy from us. The medicine itself may be for someone else.

What is the intended user's biological sex?

We ask because it affects some safety checks (for example pregnancy).

IMPORTANT - Is the intended user taking WARFARIN, or any other anticoagulant ("blood thinner")?

This includes warfarin, acenocoumarol, and the newer blood thinners such as apixaban, rivaroxaban, edoxaban, dabigatran, or heparin.

Is the intended user taking any of these: a diabetes tablet taken by mouth (such as gliclazide or glipizide), phenytoin (for epilepsy), or a statin (such as simvastatin or atorvastatin)?
Is this for oral thrush (white patches or soreness in the mouth), or for a fungal skin infection such as athlete's foot or ringworm?
Is the oral gel for a baby under 6 months old?

Daktarin oral gel can cause choking in very young babies, so it is not for use under 6 months.

Does the intended user have liver problems?
Has the intended user ever had an allergic reaction to miconazole, or to another antifungal of the same type (an "azole")?
Is the intended user pregnant, trying to become pregnant, or breastfeeding?
Do you understand to use it as directed, and to see a pharmacist or GP if it is not better within 7-14 days or it gets worse? (For the oral gel: hold it in the mouth, and do not put it to the back of the throat in young children.)
